According to this interview it was in the script. The special effects people had actually made some kind of rig to get the pizza up there, but Bryan Cranston was able to fling it up there himself.

On friday, I learned that it's a tricky thing to give someone enough of whatever it is they give you when doing scoping procedures. I also learned what it feels like to have a tube shoved down my throat so far that it gets to my gall bladder area to yank out some escaping stones from surrounding ducts. And that when you vomit like that it comes out bright banana yellow.

Oh, medical progress. I remember when my mom had gall stones, probably like 1987 or so, and so they removed her gall bladder via an almost c-section sized scar on the right side of her abdomen.
