Okay, so with yesterday's launch of Phase 3 and the Post-Prelude login screens, every game (with the exception of AaAaAa, but I'll get to that in a moment) has been updated with one or two more Potatoes. During the prelude stage, there was one potato for every game for a total of 13.
In my sleep deprived state, I drew out a crappy, mostly illegible visual diagram showing off the potatoes for each game.

P2 refers to prelude potatoes. P3 refers to post-prelude potatoes. I forgot to add em to the last 3 'cause I'm tired as hell.

Many games on the wiki offer detailed walkthroughs or videos explaining how to access each login terminal.
Now as for the post-prelude login terminals, compared to the prelude terminals, these are much harder. Many of the first set screens could be accessed very simply, or by bypassing a challenge using a modded save file or cheat. The second batch are more time consuming, challenging, and are harder to bypass. For instance, Bit.Trip.Beat require passing a new short song, while Amnesia and The Ball have outright full on new levels to beat.
From what is out right now, there are a total of 29 potatos. However, AaAaAa doesn't yet have a new terminal. Seeing how it's the only game without a post-prelude potato, it'd only make sense that simply no one has found it yet. That'd also bring the total potato count up to an even 30.
Now, as for what these potatos mean. Based on the effort and time needed to collect all 30, I have a few guesses what this could lead to.
A) Pre-release of Portal 2 for those who collect all 30
B) New Portal 2 content (such as skins or whatever)
C) Another damn TF2 hat
All I'm hoping is that it's worth the effort. I'm up to 23 right now missing the (all from post-prelude) potatoes from Kick It, Amnesia, Audiosurf, Cogs, Rush, and AaAaAa.
And now that that's all explained, time to go crash til 3pm.

Okay, 2pm actually. Another login screen was found for Defense Grid. Ahahah's phase 3 potato hasn't been found yet, but when you start the game, there's a portal button on the lower left corner that brings you to a console.
