They're not bugged. Certain achievements are retroactive, as the game already had the capability for tracking specific stats. Such as, Most Healed in One Life, Total Healed, etc. So Intern, Specialist, and Chief of Medicine can be unlocked as soon as you log on and authenticate and all that jazz.
